Star Awards cardholders can now download their Win/Loss Statements directly from the Ameristar.com web site.

If you have an account password click here. Log on, click WIN/LOSS FORMS and follow the simple instructions.

If you do not have a password, click here and fill out a short activation form. You will be able to choose your own password and access your account immediately. Then, just click WIN/LOSS FORMS and follow the simple instruction.

In order to release information regarding your tracked play at Cactus Petes and/or The Horseshu, you must submit a signed W-2G/Win-Loss Request Form.

You may also request a copy of the Win-Loss Request Forms by calling Carrie Eslinger at 775-755-6001. Forms are also available at the Plateau Player's Club booth.

Please read the form carefully and fill it out completely and legibly. By clicking on the form fields with your mouse, it is possible to fill out the form in your browser, however you will still have to print out the form and deliver it (in person, by fax or by mail) to Cactus Petes/Horseshu for processing. E-mail and on-line processing of requests is currently not available for these properties.

Please note: if you choose to submit your request via fax or mail you will be required to obtain a notary public signature in order to verify your identity. If you choose to present your request in person, you will need to have proof of identity. Please allow up to 2 weeks for your request to be processed.
